All of the forms listed below must be completed to participate in athletics at Redeemer. The forms are PDF format. (Get Adobe Reader to view and fill out the form here.) You may fill out the forms on your computer, then print, and sign them. If you'd rather, you may print the forms and fill them out by hand. All forms are to be returned to the Upper School office.

Lunch Forms
Below, you will find ordering information for Good Taste Catering, Pizza Lunch, Hot Dog Lunch, and Chick-Fil-A. All Good Taste Catering orders are done on-line. We provide lunches as a service to parents and not as a fundraiser for the school. The exceptions to this are the Pizza Lunch, which entirely funds Grammar and Upper School field day and supports our Athletic Program, and the Hot Dog lunch, which funds the 6th grade field trip to Ellis Island.
